A simple start
READY TO PLAY?
Complete a short registration form, submit the appropriate waiver, and we'll typically review your completed registration within 24 hours.

Sign Your Waiver
Submit the appropriate signed liability waiver before your first practice.
Get Approved
Organizers review completed registrations, typically within 24 hours after receiving all required information.

Receive BenchApp practice emails. Respond right from your inbox and get back on the field.
Download your liability waiver
Download and sign the appropriate PDF, then email it to creamcityfhc@gmail.com or bring it to your first practice. Both the player and parent/guardian must sign the minor waiver. Waivers must be received before anyone participates.
Eligibility: Adults ages 18+ may register. Players ages 16–17 may participate by invitation only, with parent/guardian permission, both required signatures, and club approval. Completing the registration form does not guarantee participation.

Who can play?
Players ages 16 and older. Adults 18+ may register openly. Players 16–17 participate by specific invitation only, with parent/guardian permission, required signatures, and club approval.
Do I need to attend every practice?
No. Attend when you can. Approved players receive BenchApp emails for individual practice RSVPs.
Do I need to install BenchApp?
No. BenchApp sends practice invitations by email, and you can RSVP directly from the email.
What if I haven't played in years?
You're welcome. Cream City FHC is designed for recreational play with a range of field hockey backgrounds.
How much does it cost?
Single-session payment is available, and you can save by purchasing multiple sessions in advance. Final rates will be shared before you participate.
Where can I get the liability waiver?
Download the adult or minor waiver in the How to Join section. Signed forms may be emailed to the club or brought to your first practice. For minors, both the player and parent/guardian must sign.
How long does approval take?
Completed registrations are typically reviewed within 24 hours. Waivers and any required parental permission must be received first.
Where do we practice?
Our initial practices are at Delta Sports Performance, 2920 W Vera Ave in Glendale, Wisconsin, from 7:30 to 9:00 PM on the dates listed above.
